Retatrutide is an investigational triple agonist currently in phase 3 clinical development, targeting GLP-1, GIP, and glucagon receptors simultaneously. This multi-receptor engagement is hypothesised to produce additive or synergistic effects on energy balance, glucose metabolism, and body composition beyond what dual agonists achieve. Early-phase trials have shown substantial weight reduction and HbA1c improvement. With a half-life of approximately 144 hours and a Tmax of around 36 hours, it is designed for once-weekly subcutaneous administration. As an investigational compound, its long-term safety profile is still being characterised, making systematic side-effect and dose tracking especially important for early adopters enrolled in clinical protocols.

Mechanism

An investigational triple agonist targeting the GLP-1, GIP, and glucagon receptors. The added glucagon-receptor activity is hypothesized in research to increase energy expenditure alongside the incretin effects on appetite and glycemia.

Research summary

Investigational and not approved by the FDA. Phase 2 results for obesity and related conditions have been published; larger phase 3 evaluation is ongoing per the literature.

Reported side effects

Early trials report dose-dependent gastrointestinal effects and, consistent with glucagon activity, monitoring of heart rate and metabolic parameters. The full safety profile is not yet established given its investigational status.
